Refer Child for Appointment of Child Advocate

The Young Center accepts referrals for the appointment of Child Advocates. Please follow these steps and all referrals will be submitted directly into our secure database:

• Click here to be directed to the referral website. Please note you do not need to sign in or register.

• Click on “Start Referral.”

• Fill out all required fields (indicated by a red star).

• To print/save a copy of the referral before submission, click the print icon on the top right corner of the screen BEFORE clicking “Save”.

• When you are done, click “Save” in the lower right-hand corner of the screen and the referral will be submitted.

• Thank you for submitting a referral to the Young Center. We will review your referral and assess our capacity to assign a Child Advocate. We will be in touch if we have any questions.
